Proposed Effective Date - The first of the next month following receipt in the CALS HR Office
Proposed Base Adjustment - Specific dollar amount (no cents). Indicate the 100% rate regardless of appointment percentage
% Increase - up to the minimum of range and 5% - 10% over that. May request an increase of more than 10% if there has been a change in duties and also a title change. Academic Personnel will review extraordinary situations. A written justification will need to be included when requests are submitted outside of the standard criteria.

Title Code - Consistent with title requested
Proposed Salary - Current rate plus adjustment.
