Completion of Rate and/or Title Change Form



  • Employee's Name - as stated

  • Appointment ID - specific to this request

  • Department/Division - specific to employing department making request

  • UDDS - specific to employing department making request

  • Current Salary - as stated - 100% rate regardless of appointment percentage

  • Proposed Effective Date - The first of the next month following receipt in the CALS HR Office

  • Proposed Base Adjustment - Specific dollar amount (no cents). Indicate the 100% rate regardless of appointment percentage

  • % Increase - up to the minimum of range and 5% - 10% over that. May request an increase of more than 10% if there has been a change in duties and also a title change. Academic Personnel will review extraordinary situations. A written justification will need to be included when requests are submitted outside of the standard criteria.

  • Proposed Title - specific to request

  • Title Code - Consistent with title requested

  • Proposed Salary - Current rate plus adjustment.
