CALS Hiring Restrictions: Offer Requests



Offer request are required for all positions that are deemed essential and that have received approval to proceed with recruitment efforts. Requests to proceed with an offer will need to be submitted for Dean/Dean Designee review to receive CALS approval. This second review is required on all State- GPR/tuition funded (101 & 104), and Funds 118-5, 136, 176, 402, and 133/150 indirect cost returns.

Checklist for Offer Request:


  1. Send an offer request to offerrequest@cals.wisc.edu
  2. Please include the offer PVL# or CHRIS# and position type in the subject line (e.g., Offer PVL/CHRIS# and new or vice- Last Name)
  3. Offer request should include the following information:
    • Offer amount
    • Position title
    • Position start and end dates
    • Applicant resume, e-mail address, and DOB (For individuals without an e-mail address, include a complete mailing address.)
    • Justification statement
    • Target date by when the Unit wishes an offer to be made
    • Complete funding string for the Criminal Background Check to be billed to.

*Note: If a letter of offer has been drafted by the unit, it can be included for review at this time. All letters of offer should be clearly marked as Draft until final.

Background Checks

UW System and UW-Madison policies on criminal background checks include an expectation that criminal background checks will be completed prior to an offer being made. If the offer request is approved, CALS HR will proceed immediately with the background check.

Units should tell all applicants that in order for an offer to made, a criminal background check will be performed and that they will be contacted by CALS HR to secure their permission to conduct the check.

If a background check cannot be completed prior to an offer being made, the following statement must be used in the appointment letter: "This offer of employment is conditional pending the results of a criminal background check. If the results are unacceptable, the offer will be withdrawn or, if you have started employment, your employment will be terminated."
