IDs For Retirees

Pam Baumen in APO processes applications for UW IDs and continued email for all retirees that do not request emeritus status.

How to request ID:

Give the following information to Pam Baumen (3-2511, pbauman@bascom.wisc.edu):

1. Name of retiree
2. Social Security Number of retiree
3. Date of birth
4. Gender
5. Permanent home address
6. Let her know if retiree wants his/her permanent home addressed released (for purposes of future directory)

If the retiree is not requesting emeritus status, the department should request retirement certificates through CALS Human Resources.
