Medical Records
POLICY FOR MAINTAINING MEDICAL RECORDS
- All medical information must be kept in a separate locked file that is maintained by
CALS Divisional Disability Representatives (DDR's)
- This information will not be included in a Personnel File in the departments, CALS Administration/Human Resources Office, or the Classified Human Resources.
- If anyone in the departments other than the DDR's receive medical information, they need
to forward this to CALS DDR's.
- FMLA, ADA, Leave Without Pay Requests, Income Continuation Insurance or other benefit
forms may be kept in the departments, but will need to have any medical information eliminated.
- CHR will not maintain personnel files in their office that contain any type of medical information.
Leave of Absence forms that contain medical information needs to be sent to them with the specific
medical information removed.
- We recommend that employees provide medical information on a separate
sheet from any required forms.
