Information regarding the purpose of each form:

  • Expectations & Goal Setting: to be completed within the first month of a new appointment. Continuing employees will generally complete Expectations & Goal-Setting as part of their Summary (annual) Evaluation form.
  • Probationary Midpoint Conversation: to be completed at the midpoint of a probationary period.
  • Probationary Summary Evaluation: to be completed at the end of a probationary period.
  • Performance Management Midpoint Conversation: to be completed at the midpoint of an annual evaluation period.
  • Summary Evaluation: to be completed at the end of the evaluation period and at the end of each evaluation period thereafter.

Note: Units within CALS are expected to use these forms unless a unit receives written permission from CALS HR to use an alternate form (which CALS HR will ensure meets the policy).

Performance Management Cycle:

The normative CALS Performance Management cycle will be as follows:

  • Annual Summary Evaluations: completed in the beginning of the year (January – March)
  • Mid-point conversations: completed in the second half of the year (July – September)

Note: Units within CALS are expected to follow this cycle unless a unit receives written permission from CALS HR to follow an alternate cycle (which CALS HR will ensure is conducive to the completion of the required forms).

Compliance: As the policy states, the consequences for non-compliance are as follows: “In order to be eligible for general wage adjustment (GWA) pay plan or performance pay increases for the fiscal year, supervisors and managers must complete all required performance management activities for those employees whom they supervise, in accordance with the requirements of this policy.”
