Request for Wireless USB Modem for Travel or Special Assignment Purposes

1. Send an email to:  acs@cals.wisc.edu  
2. Include the following information:

• Name
• Department
• Begin/End Date of travel (please allow 2-3 days notice since use of the Verizon Modems will require software to be installed on the employee’s laptop)

3. Someone in ACS will then contact you to arrange for software install (if needed) and delivery of the Verizon modem with instructions on how to use the modem.
4. You are expected to return the modem to ACS upon your return to the office unless other arrangements are made.
