Authorization Information

• InfoAccess Authorization
  

  You must purchase InfoAccess from the DoIT Tech Store which will include an InfoAccess Authorization card. This card must be completed and returned to DoIT for processing prior to trying to access any InfoAccess Data Views.

• ISIS Authorization
  

  If you have never been granted access to the Integrated Student Information System (ISIS), you must submit an Access and Compliance (FERPA) form along with the authorization form. [No access will be granted without a signed compliance form.]

• Extended Access to ISIS Authorization
  

  Please be aware of the UW-Madison ISIS Extended Access Policy.
  In addition, CALS has developed a Supplemental CALS ISIS Extended Access Policy.
  CALS requires a written justification which must be included with the request form.

Authorization Process (applies to ALL authorization forms)

• After completing the authorization form, sign and date the form

• Route form to supervisor for date and signature

• For those forms requiring Dean & Director signature and FERPA forms, route form to: Charlene Krembs, 240H Ag Hall

• Expect up to 2 weeks for the entire authorization process
